The Practice
Herald Avenue Family Practice strongly believes that the relationship between a patient and their health provider is based on a two-way understanding, acknowledging that both have a responsibility for one another. We endeavour to provide the best possible health care to our patients.
Patient Rights and Responsibilities
Our policy is that we treat all cultures with respect and encourage a two way learning process to enable the best possible medical health care for the local community. We encourage patients to identify with their cultural status to us so that we can do our best to offer any specialised services available
Wheelchair access and toilet facilities are provided at Herald Avenue Family Practice. There is a facility to place distressed patients in a private room while that are waiting to see the doctor.
Services
General services available include management of all aspects of child, adolescent and adult health.
Other services provided include:
- Repair of lacerations,
- Removal of skin lesions,
- Removal of corneal foreign bodies,
- Child, adult and travel immunisations,
- Management of uncomplicated fractures,
- Management of work related and motor vehicle accidents, including the management of injuries and liaising with rehabilitation providers and employers.
Our medical instruments undergo full cleaning and sterilisation procedures in accordance with Australian standards.
Results
Please phone 9259 5559 Monday to Friday between 2:00 pm and 3:00 pm for information regarding test results, unless advised otherwise by the doctor. Usually allow three days for the result to be checked by the doctor. The only information given by phone regarding tests will be relayed comments from the requesting doctor. Patients are advised to make follow-up appointments to discuss the test results with the doctor, unless advised by the treating doctor that it is not required. If you wish to receive copies of test results, please discuss with your doctor at the consultation, and he can arrange for it to be sent directly to you.
Recalls and Reminders
We have a system to recall people who are due for review, such as for diabetic patients, PAP smears or colonoscopies. Please let us know if you would like to be included in this system.
Contacting Your Doctor
Our reception staff can assist with most enquiries. Please understand that they will ask you information to ascertain the nature of the call and assess the urgency of the matter. Emergency medical matters will be referred to the doctor immediately. However, the doctors prefer not to interrupt consultations to take calls, as this can be intrusive for all concerned. To avoid this, patients may be placed on hold or patients advised to return calls when staff have been able to seek advice between consultations. It is important to understand that it is preferable to schedule an appointment with the doctor in person so they can provide you with the best possible care.
We encourage you to have a review when requiring a repeat prescription or referral. This is regarded as best practice medicine. At the discretion of the Doctor concerned a repeat prescription or referral may be done without an appointment.
Management of Health Information
You medical record is a confidential document. It is the policy of this practice to maintain the security of personal health information at all times and to ensure that this information is only available to authorised members of staff. Consult this notice for more information.
Patients are entitled to access their own health records at any time convenient to both themselves and the practice.
We ask that the patient's request for their medical record be in writing and addressed to the doctor concerned. We may impose a charge for photocopying involved in processing the patient's request.
